When a patient is a current member of our DCM plan, they can add a family member living in the same household to our DCFM plan at a discounted rate. With our DCFM plan, we can assure you that all medical needs will be met for your entire family.

Direct Care Membership Family DCMF (members living in the same household as a current DCM patient )
(DCMF) Add on $120 upfront ( covers you for 3 months)
Direct Care Membership Family monthly fee $40
